Fail with better diagnostics output

An example of a fatal error which doesn't give very good indication of where things go wrong is the ENOMEM issue when we're out of stack space while creating lots of threads (https://postlister.uninett.no/sympa/arc/radsecproxy/2010-03/msg00025.html).
